Case study Part one: Write a case study on an adult with dementia you have recently cared for. Your case study should include all key relevant clinical information and demonstrate advanced nursing practice that includes assessment, therapeutic engagement, and care planning inclusive of the socio-cultural perspective. Part two: Choose an “issue” relevant to the person in the case study and using the academic literature and your reflective skills discuss how you would provide the best outcomes for the person. You will be required to present the brief overview of your case and the issue/s you identified to the class online at the end of August. Guidelines to assist in preparing and writing the Case Study: Assignment 1. You can discuss and write up the case either in essay form or by headings. When describing the type of dementia your patient has been diagnosed please include some detail around the neuro anatomy of the brain and discuss the common or not so common signs and symptoms this client might present with to support the differential diagnosis supported by evidence and the literature. Are you aware of any pre-existing risk factors that might suggest this patient was at risk for example hypertension etc? Did the patient undergo any special tests to reach the differential diagnosis? If so what is the evidence to support conducting these tests. What were the diagnostic criteria for the dementia you are describing? Consider the following: How does this type of dementia present in behaviour and affect. What would the typical physical findings be in the person with this dementia type? Has the patient been on any specific dementia type drugs if so what the evidence to support this? What stage of the dementia pathway is the patient presenting as mild to moderate, severe. What support your decisions? How is the person with dementia supported at home or in aged care? What might have or is supporting this person to age in place. As a nurse reflect on your role in the diagnostic and support approach you provided. The second part of the assignment is to choose one issue that you have described in the case and discuss in depth using the current evidence to support the issue.
